Application Instructions

If you are applying for help obtaining a Birth Certificate, state ID’s or other important ID’s follow these instructions:

Two different documents showing proof of current Ohio residency:

Acceptable Documents (dated within 3 mo):

  • Child support statement
  • Bank statement
  • Credit card stub
  • Utility Bill
  • Insurance policy

Information about OHIO IDENTIFICATION CARDS

In Ohio, an individual is not permitted to hold an Ohio driver’s license and an OH Identification Card at the same time. Having an ID card issued will cancel a driver’s license requiring the applicant to retest if/when he/eh applies for a new driver’s license.

    Is your current legal name the same as the name listed on your BIRTH CERTIFICATE, PASSPORT, or document(s) issued by the US DHS (Department of Homeland Security)?

    If the answer is NO – You must connect your birth name with your current legal name. You must bring an ORIGINAL or CERTIFIED COPY of the following name change documents to make that connection:

    • Marriage Certificate or Marriage License*
    • Certified Copy of a Decree of Divorce, Dissolution, or Annulment of Marriage*
    • Certified Copy of a Court Ordered Name Change

    *NOTE: If you have had more than one marriage, you may need to bring documentation from each marriage and/or divorce to connect your birth certificate, passport or US DHS documents to your current legal name.

  • Please note: If you request is approved, you will need to arrange to go to St. Aloysius Catholic Church, 150 S. Enterprise St. to pick up a check for the Birth Certificate, State ID, or other ID.
